Appointment Checklist
On the day of your appointment, be prepared by obtaining your previous treating Physician's medical records and all diagnostic test films. You may be referred for updated x-ray or other diagnostic tests deemed necessary. This will aide our Physician in diagnosing your condition and determining the proper treatment plan.
We suggest you print this as a reference for your first visit.
Medical Records
- Those records related to your present orthopaedic condition will be most helpful.
- Gather all information possible.
X-Ray Records
- All X-ray films taken within the last six months, including MRI's, Bone or Cat Scans.
Medication List
- A list of all your current and over the counter medications and vitamins.
- A list of any known drug allergies.
Identification Required
- Bring your Insurance card(s)
- Bring your Photo Identification (ie: Driver's License)
Be Prepared to Discuss Your Condition
- What happened to precipitate your condition?
- Is it getting worse?
- How does it limit you form performing your normal daily routines?
- Have you had prior orthopaedic injuries or orthopaedic surgical procedures? If so, when?

- We suggest you write notes and questions to discuss with the doctor on the day of your appointment.
