Accepted Insurance Plans

Below is a list of major insurance plans we accept. We also accept most Workers Compensation plans. If you do not see your insurance company listed, do not hesitate to contact our office to inquire (select General Questions or Office Appointment Questions.)

If your insurance requires a referral or authorization number, it is your responsibility to obtain from your primary care physician. This referral or authorization number is required for your initial orthopaedic evaluation and treatment (ie. surgery, therapy, diagnostic study, DME).
